Our mission is to provide service dogs to those who would benefit from partnering with them.
We use a multi-step admission process to allow us to learn more about your needs and for you to learn more about our organization and our dogs. On average, this process takes 4 to 8 months to complete.
If you would like to begin the process for a Brigadoon service dog, please submit the following eligibility questionnaire using the links below. Please note that this is the first step in our admission process. This form is not the written application. Your questionnaire will be reviewed within 4-6 weeks.

For this admissions cycle, eligibility is limited to:

Applicants living in Washington, Oregon, Idaho, or British Columbia

Applicants 16 years of age and older. Those under the age of 18 must have a legal guardian sign the Eligibility Questionnaire.

If you are deemed eligible, you will be provided with a more detailed written application to fill out. You will also receive additional forms for your healthcare providers to complete to supplement your application.
Your application is not considered complete until we have received:

A $50 nonrefundable fee (This fee is waived for veterans)​

Dog Application
Please complete this application if you are interested in a released dog. We keep your application on file for one year.
While we sincerely wish that we could help every qualified applicant and we aim to expand the number of clients we serve, please be aware that we receive far more inquiries and applications than we have dogs to place. Not all applicants who meet the eligibility requirements will be selected to move forward in our process or accepted as clients.
